Some key items:

	The appropriate licenses and appointments must be in place. Visit our Licensing page for more details.
	Premium must be received on or before the effective date.
	Premiums are to be submitted as gross.
	Accident Claim forms are available in both English or Spanish. Visit our Claims page to download the appropriate carrier form.
